How to fill out the following ACORD forms: Contractors Supplement, Professional Liability Supplement, Restaurant/Tavern Supplement
All of these ACORD forms are located on the Business Owners Package policy. Open up this policy and go to the tab called "Spec Prog/Prior Policy". There is a button for each of these forms. This will always open up a new form.

To open a form that was previously filled out, go to the "Apps" button on the banner. Put the dot next to Forms-Non Apps. Highlight the appropriate form and click OK. It will pull up a locater of the forms that are already filled out. Type in the client''s name and click OK.

At this time there is no way to open these from the client screen. However, they can be emailed from the client screen.
